The proc is based upon being hit (which if you are a tank will be often) the Madness card only procs if you land a killing blow (Which is quite uncommon as a dungeon or raid tank). Also the proc for the Madness card is as often as not not somthing that a melee class can use. (it is a random proc.) The Madness card is much more suited to a dps class. As a result I've got the vengeance card 1 week after I hit 70. It's really good stuff, costed me a total of about 1.7K gold. I'm an engineer, using a 45 sta trinket plus tankatronic... Tons of HP!
